Ahead of your visit, please gather copies of any previous evaluation reports, including neuropsychological evaluations, school-based assessments, and speech/language or occupational therapy evaluations. Please also provide us with a copy of any current IEP or 504 Plans, and feel free to upload your pre-appointment paperwork if complete.
